What do I need to bring?

• Your insurance card, if applicable
• Your photo ID
• A medical release form, if you are under 18 and unaccompanied by a parent or legal guardian

How long will my visit take?
Most Hometown patients are seen in 30 minutes or less and treated in under an hour.

Do I need an appointment?
No! Hometown Urgent Care offers convenient extended evening and weekend hours 365 days a year, no appointment necessary!
